Facilities: Interim Storace Area .

Attached is a sketch of the interim storage area where gauges will be stored when notin use. Tho' equipment room is secured by locked doors.' The equipment manager hasthe equipment room key'and controls access to the room so that unauthorized persons. may not remove equipment from the room. The Troxler gauge will be further protected -when not in use by chains around its case which will be secured by a lock to the wall.The equipment manager controls the key to the lock on the chain, and ensures that only

,

individuals approved by the Radiation Safety Officer are_ allowed access to the gauges.The doors on the wall opposite the garage door are kept locked.

The distance from the gauges to the nearest work station is more than 15 feet in everydirection. The area on the far side of the back wall is an uninhabited open area. No-parking is allowed in the open area.

2. Operating Procedures

A. Transportation of Equipment

1. All possible mearm shall be provided to ensure that the equipmentsecured in the transporting vehicle and the equipment is away frompassenger compartment. When transporting in a;. anclosed vehicle(van), the vehicle will be locked. When transporting-in an openvehicle, the gauge should be'secuiely fastened and locked to thetruck bed.

2. The gauge will be transported in the transportation case. TheDepartment of Transportation requires that the gauge be transportedin a properly labeled carrying case.

3. At all times during transport, the operator will- have a properlycompleted Bill of Lading for each gauge.

B. Utilization Procedures

1. When the gauge is in the field, the auth.orized user must maintain.control over the gauge at all times. The gauge must never be left'unattended.

2. When not making measurements, authorized personnel should place -the gauge in the transportation case and return it to its permanent

|- storage area as soon a_s possible. The gauge is to _be used for its- intended purpose only. By doing so, you will minimize any radiationexposure to as low as reasonably achievable (ALARA).

equipment, your monitoring device is to be stored in the radiation--. free area that has been designated in the office.

C. Maintenance and Leak Test Procedures.

1. Periodic maintenance will inclu'de cleaning the gauge During anymaintenance, you must wear your personnel- monitoring device.Accepted cleaning and lubrication procedures developed by themanufacturer will be followed.

.

2, No maintenance will be performed in which the radioactive source isremoved from the gauge. For this type of maintenance, the gauge -will be returned to the manufacturer.

3. The leak test will be performed using the Troxlec Model 3880 LeakTest Kit and the manufacturer's instructions. Again, the personnelmonitoring device will be employed. Gauges will be leak-tested at.intervals not to exceed six months.

.

D. Record Keeping in Regards to Facility Decommissioning

Records of information important to the safe and effective decommissioningof the facility will be maintained in an identified location,- until the license is -

' terminated by the Commission. The file shall be entitled " Facility-Decommissioning File." The records willinclude the following:

'

1. Records of any leakage involving the spread of contamination, wherethe contamination remains after clean-up procedures' have beenexhausted, and/or if the contamination is inaccessible.

2. Drawings or sketches of areas in.the facility where radioactive*

! materials are used and/or stored. These drawings will indicate -locations of any non-ramovable' contamination.

,

3. Records of the cost estimate for the decommissioning of the facility.

3. Emergency Procedures:

A In the event of physical damage to a gauge, the following will be performed:

1. Immediately co_rdon off an area around the gauge.- An nrea radius -of 15 feet will be sufficient.

.

2. If a vehicle is involved, it must _ be stopped until|the extent'of -_

contamination, if any, can be established.

3. A visual inspection of the gauge is to be made to determine if thesource housing and/or shielding has been damaged.

4. At the earliest possible time, when the situation is under control, you,

must contact Joe Hicks at (804) 358-5400. Describe the presentconditions and follow the instructions of the Radiation Safety Officer.

B. In the event the gauge is lost or stolen, immediately notify the RadiationSafety Officer, Joe Hicks, e' (804) 358-5400.

- Item 11 - Waste Management

Disposition of the gauge will be by transfer to either another licensee specifically licensedto possess the radioactive material or to a licensed disposal facility. The manufacturerwill assist in locating a properly licensed disposal facility.
