Radiothérapie Interne Vectorisée «RIV»Targeted Radionuclide Therapy «TRT»
• Administration d’un radiopharmaceutique pour irradier les cellules tumorales
• Prérequis : Un rapport de captation tumeur / tissus sains très élevé(augmenter l’efficacité et minimiser la toxicité)
• Se distingue de la radiothérapie externe par :
– Traitement systémique avec capacité de ciblage de l’ensemble des métastases, y compris les mico-métastases occultes
– Irradiation délivrée à bas débit de dose, sur une durée qui dépend de la demi-vie effective du radiopharmaceutique au sein des tumeurs
– Une hétérogénéité de distribution peut limiter l’efficacité (fibrose, nécrose, perte d’expression de la cible tumorale, …)

Whole-body scans(posterior views)
• First scan (left) obtained at the time of 131I remnant ablation (100 mCi, off-T4 Tg = 22 ng/ml). In addition to uptake in the thyroid bed (masked), focal 131I uptake is visible on at levels of T6, L2, right scapula, and on the left iliac bone. Morphological imaging and 99mTc-HMDP bone scan are negative.
• The 131I scan on the right was obtained at the time of the third course of 131I therapy (Tg = 4.2 ng/ml). This scan shows no residual 131I uptake at metastatic sites (physiological uptake in stomach).

Benign focal uptake related to asymmetric radioactivity in the submandibular salivary glands after total thyroidectomy in a 49-year-old woman with papillary thyroid cancer.
Transient or permanent narrowing or obstruction of the salivary duct may cause this pattern of asymmetric radioiodine accumulation often associated with previous 131I therapy.

Particularités des TNE-GEP
• Traditionnellement classées en:• Foregut: pancréas, estomac, duodénum • Midgut: iléon, appendice• Hindgut: colon, rectum
• Parfois fonctionnelles (gastrinome, insulinome, VIPome, Glucagonome, ACTHome,
ou tumeurs carcinoïdes du grêle avec métas hépatiques).
• T. primitive parfois difficile à détecter- petite taille- multifocalité- atteintes d’organe variés ou localisation difficile (organes creux)
• Diagnostique souvent tardif : Souvent métastatique au diagnostic

• Myelodysplasie : 4 patients
• Hepatic toxicity: 3 patients
• Severe renal failure: only 2 patients

• 30 patients inclus• 2 cures (93%) / 3 cures (80%) / 4 cures (47%)• Activité moy : 7,5 GBq/cycle
• Suivi médian: 25 mois

Discussion

• Meilleur résultat que Cabazitaxel (39%, TROPIC trial)

• Effet antalgique rapide et efficace : réduction chez 37% après 1er
cycle vs 9% pour Cabazitaxel (TROPIC trial)
• Pistes à venir :
– Nécessité d’essais randomisés contrôlés évaluant le Lu-PSMA versus ttt standard (cabazitaxel ++) : essai phase II en cours
– Quelle place dans l’enchainement thérapeutique ? Avant l’apparition de l’hormono-résistance ?

Reasonable treatment corridor between :
• Insufficient treatment response (red: patient died before reevaluation; orange: PSA progression; green: imaging or PSA response) • Intolerable toxicity (red: xerophthalmia; orange: xerostomia G2; green: no or only G1 xerostomia).
• During repeated therapies, insufficient PSA response was observed at 4-mo interval.
α radiation
• α-emitters produce densely ionizing high-LET radiation, causing complex DNA damage leading to prominent cell killing due to unrepaired damage.
• They are less influenced by cell cycle or oxygenation state and can counteract previous resistance to conventional radiotherapy and chemotherapy
• 225Ac emits 4 α-particles and has a half-life of 10 days that fits with the long retention time of PSMA ligands in tumor tissue
• However side-effects are more frequent and appreciation needs longer follow-up.
