Read the Asphalt Plant General Order. You can get it online at www.ecy.wa.gov/programs/air/AOP Permits/Boiler/GeneralOrders.htm, or call the appropriate regional office (see below) for a copy. Answer all the questions and sign and date the application. Enclose manufacturer specification sheets for the asphalt plant, including dryer drum, baghouse, burner, and power generators. Enclose a check to the Department of Ecology for the application fee. State Environmental Policy Act (SEPA) Compliance lif$875 application fee if SEPA review is complete - Include a copy of the final SEPA checklist and SEPA determination (e.g. DNS, .MDNS, EIS) with your application. $1,160 application fee if SEPA review is required - If SEPA review has not been conducted, please fill out a SEPA checklist and submit it with your application. Knife River Corporation Northwest respectfully submits the included General Order Application for a Portable Asphalt Plant The initial location is described as SEfrac14 of NEfrac14 Section 23 Township 22N Range 39E and is an active Washington DOT aggregate source (Fishtrap Knife River assumes that a SEPA checklist and determination has been completed for this site

Knife River did not supply serial numbers for the generators in the application Knife River operates 3 like kind portable asphalt plants using like generators Knife River would like the flexibility to place any of the three like plants at the location and associate the plant with the WA permit Serial Numbers can be supplied if required
