dlgations rgionales des enseignements secondaires et lEcole Nationale Suprieure

Polytechnique ou tlcharger sur Internet : www.fgi-ud.orgA registration form to be collected from the deanery, regional delegations of secondary

education, at the Higher National Polytechnic School or on the site: www.fgi-ud.com.

This form shall bear a 1000 CFA F fiscal stamp and be duly completed and signed by the

candidate;

Une copie certifie conforme de lacte de naissance datant de moins de trois mois et

signe par une autorit administrative ou municipal; A certified true copy of birth certificate not older than three months and signed by

administrative or municipal authority;

Une copie certifie conforme du Baccalaurat ou du GCE/AL datant de moins de trois

mois ;A certified true copy of Baccalaureate or GCE/AL not older than three months;

Une copie certifie conforme du Probatoire ou du GCE/OL datant de moins de trois
