Depuis cette date, le secteur soin et aide à domicile se développe pour proposer aujourd’hui / since that time, the homecare and support sector is developing its activity and offers today:

L’Equipe Mobile Alzheimer (EMA) / The Alzheimer mobile team

• Ce dispositif est créé dans le cadre du plan Alzheimer 2008 – 2012 qui prévoit une prise en charge adaptée, pour permettre au malade de continuer à vivre chez lui / created in the frame of the French Alzheimer plan 2008-12; adapted care in order to allow the patient to still stay at home

• Ce dispositif doit / it has to be : – s’inscrire dans un projet de soins et d’accompagnement /written in a

care and follow-up project, – être dispensé par un personnel formé / delivered by a trained person, – mobiliser des compétences pluridisciplinaires / mobilize

multidisciplinary competences.• Ce dispositif doit obligatoirement être adossé à un SSIAD / it

has to be complementary to a home nursing service.

Des objectifs multiples / a variety of goals

• Maintien et stimulation des capacités restantes / stimulation and keeping up the remaining skills,

• Apprentissage des stratégies de compensation, learning the compensation strategies,

• Diminution des troubles du comportement / reduction of the behaviour troubles,

• Amélioration des capacités patient-aidant, improvement of the skills patient-carer,

• Adaptation de l’environnement de nature à maintenir ou à améliorer le potentiel cognitif, moteur et sensoriel du malade ainsi que leur sécurité / adaptation of the environment in order to keep or to improve the cognitive, physical and sensory potential of the patient and their safety.

Une réunion d'équipe est organisée toutes les semaines pour faire le point sur / a team meeting is organized each week to

evaluate the situation about : » les nouvelles prise en charge à commencer /the new cares to

begin, » Les difficultés sur les prises en charges en cours /the difficulties

around the on-going cares, » Les bilans de fin de prise en charge /the evaluation at the end of

cares .

Critères de prise en charge / care criteria

• Etre diagnostiqué Alzheimer ou maladie apparentée /to have a diagnosis of Alzheimer or similar disease,

• Présenter des troubles cognitifs à un stade léger ou modéré / to show cognitive troubles et a light or moderate level,

• Conserver une mobilité, une capacité d’attention, une capacité de compréhension/ to keep a physical mobility, attention and understanding skills

• Vivre à domicile ou dans un établissement d’hébergement qui n’est pas un EHPAD/ to stay at home or in an institution different from a specialised medical institution

Déroulement d’une prise en charge/ development of a care pathway

• Prescription médicale de 15 séances de « soins d’accompagnement et de réhabilitation » / medical instructions of 15 sessions of « support and rehabilitation cares »

• Par / from : • Le médecin traitant / family doctor (GP).• Le médecin spécialiste / a specialist

– Un neurologue /a neurologist , – Un médecin de la consultation mémoire / a doctor from the

memory consultation, – Etc.

Première visite / First visit

réalisé par l’infirmier coordinateur / realized by the coordinating nurse: – Le recueil de données / collecting the information– l’évaluation AGGIR / the AGGIR evaluation

Si les troubles cognitifs du malade sont compatibles avec le cahier des charges / if the cognitive troubles of the patient match the specifications :

– l’entrée administrative / administrative entrance– Les renseignements pratiques de la prise en

charge / practical details of care

La seconde séance / second session

Réalisée par la psychomotricienne ou l’ergothérapeute (avec ou sans assistant de soins en gérontologie)/ realized by a psychomotor therapist (with or without assistant gerontologists)

- c’est l’IDE Co qui détermine qui sera le mieux placé pour la prise en charge – the coordinating nurse is the one who determines the best person for the care

– Mise en confiance du malade / the patient put as ease, – Poser un diagnostic par un recueil de données sur la base

du / diagnosis after collecting data on the basis of : • BME (Bilan Modulaire d’Ergothérapie) par l’ergothérapeute / evaluation of the

occupational therapist• ___________ par la psychomotricienne/ evaluation of the psychomotor therapist .• Réalisation de tests type MMSE / specific tests.

– Etablir un plan individualisé d’accompagnement et de réhabilitation / writing of a an individualized care and rehabilitation plan

La Troisième séance / third session

• Bilan réalisé par l’Ergothérapeute ou la psychomotricienne / evaluation from the occupational therapist or the psychomotor therapist

• But: déterminer 1 ou 2 objectifs maximum / Aim: definition of maximum 1/2 goals

– La toilette / washing and bathing ,– L’alimentation / nutrition, – Les activités quotidiennes… / daily activities...

Les séances suivantes/ the next sessions

• Avec l’aide des assistantes de soins en gérontologie, la psychomotricienne et/ou l’ergothérapeute met en œuvre les moyens visant à atteindre le ou les objectifs fixés.With the help of the assistant gerontologists, the psychomotor therapist implements the means in order to achieve the planned goal(s)

La dernière séance/ the last session

• Un bilan de fin de prise en charge est rédigé et envoyé au médecin prescripteur et/ou au médecin traitant. An evaluation at the end of care is written and sent to the family doctor and/or the specialised doctor

Exemple de conclusion d’un bilan de la psychomotricienne / Example of conclusion from a

psychomotor therapist evaluation « En conclusion, un travail a pu se faire sur l’anxiété et sur ses causes

par une approche psychomotrice permettant à Madame D de reprendre confiance en elle et dans la relation à l’autre. Son comportement s’est alors amélioré, elle s’est montrée moins opposante dans la relation de soin, plus confiante. Sur un plan cognitif, un travail a pu se faire également dans lequel Madame D a pris beaucoup de plaisir. Les rendez vous avec l’équipe ont scandé les semaines et donné un rythme et des activités qui lui ont été profitables. »

« In conclusion, a work has been done on anxiety and its reasons thanks to a psychomotor approach which allowed Mrs D to find her self confidence again and to trust in the relation with others. Her behaviour shows then improvement, she showed herself less opposing in the care relation, more confident. At the cognitive level, a work could be done too where Mrs D had a lot of fun. The appointments with the team marked out the weeks and gave a rhythm and profitable activities

Mrs C would not like to take them away, those could be fixed on the ground with double face scotch tape in order to prevent Mr C to slip or stumble on it

La salle de bains étant très étroite, avec la porte ouvrant vers l’intérieur, il est possible de supprimer celle-ci et de mettre un rideau à la place pour moins d’encombrement. /The bathroom is very narrow, with a door opening to the inside, it’s possible to take it away and to install a curtain in order to have more space

Le siège devant le lavabo n’est pas adapté. Je conseille un tabouret de toilette bénéficiant d’une assise réglable en hauteur et d’accoudoirs pour faciliter le lever et effectuer les transferts en sécurité. (cf document joint). / the seat in front of the washbasin is not adapted. I advise for the washing a stool with an adjustable height and armrests which will help to get up and to guaranty safe transfers

En ce qui concerne la baignoire, une planche de bain peut convenir. Mr C pourra alors s’assoir puis pivoter avec l’aide de l’infirmière. Une barre d’appui à ventouse est également à envisager. (cf document joint)./ concerning the bath, a bath board can be used. Mr C could so seat and turn with the help of the nurse. A grab rail could be installed too.

Dans les toilettes, il est nécessaire de prévoir un rehausse WC de 9 cm ainsi qu’une barre d’appui murale, ou un cadre de toilette réglable en hauteur (cf document joint)./ in the toilets it’s necessary to have a booster seat of 9 cm and a grab rail fixed on the wall.

En ce qui concerne l’accès à la maison, la seule solution serait de mettre en place un monte-escalier. Une demande de devis doit être réalisée au préalable. / concerning the access to the house, the only solution is to install a stairlift. A request for an estimation has to be realized first
