Change Management Lessons Learned
Optimization of EMR Use Across
Alberta for a Provincial Improvement Initiative
www.topalbertadoctors.org
HALIFAX, July 26, 2012 - Premiers received the report of the Council of the Federation Working Group on Health Care Innovation, From Innovation to Action . . .
Recommendations . . . that provinces and territories intend to implement as they deem appropriate to their health care system, include: • promoting the adoption of clinical
practice guidelines for treating heart disease and diabetes;
* Canadian Cardiovascular Harmonized National Guidelines Endeavour
www.topalbertadoctors.org
C-CHANGE* Imperative
• Endorsed by the Council of the Federation
• Priority for Alberta Strategic Clinical Network
• 89 Recommendations – largely aimed at primary care
• Significant care gaps
• What could we do at scale?
* Canadian Cardiovascular Harmonized National Guidelines Endeavour
www.topalbertadoctors.org
Premise
• Some care gaps arise from missed opportunities for patient engagement, not physician execution
• We can improve care delivery by coaching the clinic
team
• We can improve care at scale with primary care organizations as partners and a “train-the-trainer” approach We can now confirm these assumptions
www.topalbertadoctors.org
Why the Alberta Screening and Prevention Initiative (ASaP)?
• Family physicians do an outstanding job of screening individual patients during focused screening visits
• The challenge is more than one-third of patients simply do not “self-present”
• The majority of these patients are attached to physicians
• Evidence says the most effective behavior change tool is an invite from their primary care provider and team to complete screening
• Opportunistic and/or outreach engagement works and it is time and cost effective
www.topalbertadoctors.org
The ASaP Opportunity
The ASaP Initiative is focused on supporting primary care providers ( physicians and nurse practitioners) and team members to offer a screening and prevention bundle to all their adult patients through enhanced opportunistic and planned outreach methods, targeting patients who do not present for screening care.
– Voluntary participation
Collaboratively with primary care organizations in Alberta
– Primary Care Networks were instrumental in supporting implementation of ASaP within members clinics
– Family Care Clinic – Other organizations (E.g., university Departments of Family Medicine)

How does this occur?
• Toward Optimized Practice offers training, tools and resources to the primary care organizations to support the screening and prevention improvements within member clinics – EMR Resources were identified as key supports
• Following training, primary care organizations:
– engage physicians to participate – offer practice facilitation to primary care providers and team
members to support the development of panel and customized processes at the clinic level

Alberta Primary Care EMR Landscape
• Primary care providers using EMR ~ 80% • Dominant 3 EMR solutions – majority of the
primary care space – one vendor • Minority 3 EMR solutions • Small minority of “one-off” EMR solutions
24

EMR Supports Activities
• Environmental Scan • Identification of
recommended practices – User approaches and
recommendations
• Vendor Resource Inventory – Documentation, Videos, Help
files
• Vendor relationships • Vendor user meetings
Results • Tip Sheet Documents
– Recommend vendor help files and videos
• User videos • Learning webinars • Vendor created templates • Vendor user meeting
presentations • Template sharing
– Network users • On-site or virtual support

Clinical EMR Activities
• Attach active patients to providers
• Manage patient status • Produce a list of
patients
• Record screening offers in standardized, structured format
• Use clinical decision support tool (E.g, population reminder, flowsheet)
• Reports

Change - EMR - Key Lessons Learned
• EMR challenging – as expected – Solutions existed – find and share
• EMRs have significant untapped potential – There exists substantial unused capability in the
currently deployed technology in Alberta

Change - EMR - Key Lessons Learned
“People who have participated in ASaP are now thinking about their data in a different way. As they move on to other clinical topics of interest, when they think about the EMR they're going to be thinking about how do they need to structure their data so that it can be used as a tool to inform their decisions or to inform patient care.”
