LES PATHOLOGIES ASSOCIEES
Le prolapsus : Prolapsus et incontinence sont fréquemment associés 3 prolapsus sur 4 ont une incontinence et la cure chirurgicale du prolapsus ne guérit pas l ’incontinence elle la FAVORISE .Il n’y a pas de consensus actuellement sur la conduite à tenir . On peut résumer la situation de la façon suivante :
Il existe une incontinence patente en pré‐opératoire prolapsus en place ou prolapsus refoulé il est légitime de pratiquer un geste spécifique de cure de l’incontinence dans le même temps opératoire Il n’existe pas d’incontinence démontrée il faut s’abstenir d’un geste prophylactique

Il s ’agit d ’une incontinence par atteinte du système intrinsèque avec une pression de clôture anormale de l ’urètre PCMU a un âge donné =
110 cm d’H2O – AGE +/‐ 20 %Toute pression de clôture inférieure à 30 voire 20 cm d’H2O est anormale quelque soit l’âge

Quelle est la place des injections périurétrales ?
En comparaison à des techniques chirurgicales par voie abdominales, les injections urétrales offrent des taux de succès moindre, mais sembles moins morbides (NP1).Il n’y a pas d’évaluation disponible à long terme. Pas de comparaison entre les injections et les BSU qui constituent le traitement de référence.
