• L’efficacité de toute modification doit être vérifiée à
1 mois et à 3 mois
Conclusion
• Une trithérapie de Nuke telle que AZT+3TC+ABC chez un patient en succes immunovirologique permet– de maintenir l ’indétectabilité – d’ améliorer la tolérance clinique– d’améliorer les paramètres lipidiques
(Triglycérides et Cholestérol total)– de simplifier le schéma posologique – de renforcer l ’adhérence– d’ améliorer la qualité de vie– d’épargner 2 classes thérapeutiques– d’éviter des interactions médicamenteuses
En résumé
• Switcher une IP vers ABC ou TRIZIVIR ou NNRTI entraîne une amélioration des paramètres lipidiques

• Gestion et prévention des principaux effets
indésirables:
Il est recommandé de changer de traitement chaque fois que
l’effet indésirable est considéré comme sévère ou pouvant à
terme entraîner une gêne significative pour le patient ou une
mauvaise observance
Mais certains effets indésirables ne nécessitent pas
obligatoirement l’arrêt du produit présumé responsable
( on peut proposer par exemple une réduction de posologie, un
traitement symptomatique ou une prise en charge spécifique
devant une lipo-atrophie, une lipohypertrophie, un diabète, ou
une dyslipidémie)

• Simplification thérapeutique:
Cette simplification thérapeutique consiste à substituer tout ou partie du régime antirétroviral du patient. Plusieurs simplifications thérapeutiques ont été proposées:
•Prise unique quotidienne
•Traitement compact associant 2 ou 3 molécules dans un seul comprimé
•Diminution du nombre total de comprimés quotidiens pour certains IP, lorsqu’ils sont associés au ritonavir à faible dose
les objectifs de cette simplification sont : d ’améliorer la vie du patient au quotidien, de favoriser l’observance, d ’épargner la classe thérapeutique substituée, de maintenir une efficacité immunovirologique

• Prévention des complications:
Un des autres objectifs de modifications de traitement est de réduire les complications à long terme, notamment cardiovasculaires, dont le risque est probablement en partie lié à la survenue d ’anomalies glucido-lipidiques
Quand le traitement initial comprend 2 IN + 1 IP ou 2 IN + 1 INN, si celui-ci est bien toléré et virologiquement efficace, il doit être maintenu pendant au moins 1 an avant d ’envisager un changement. Il est alors possible de modifier le traitement dans une logique de simplification et/ou de limitation des effets secondaires à moyen termeMais il est tout d ’abord recommandé:
de ne pas modifier le traitement antirétroviral des patients en succès immunovirologique,
sans raison dûment validée et après avoir pris en compte tout le passé thérapeutique du
